This form is to be completed for any Safe Church concern in accordance with the GBC Safe Church Policy and the Procedure fo Responding to Child Protection Concerns (p.28 of the GBC Safe Church Policy). Please do not discuss the concern with anyone other than the Safe Church Team or your Ministry Leader.

This documentation is to be kept in a locked filing cabinet and/or in secrue electronic format for at least 45 years from the date of completion.
